The very best way to remove the pillow insert is to use a Dremel tool and a stone spade bit. The bit is safe because it is smaller than the original hole drilled in the spa.

Carefully grind away the fastener by staying centered when grinding, guided by the molded hole as your center guide. As the spade shaped bit grinds deeper, it weakens the walls of the fastener. The fastener May then start to spin. When it does, this tells you that the teeth on the outside have lost their pressure. The fastener may pull out onto the spade bit, but if not, it is weak enough where you can now apply pliers to the edge of the fastener and carefully work it out with left and right twist motions as you are pulling.
